Random blog day boo yah. As you should know Fridays is random blog day were we talk anything and everything and pretty much run with it. Today's blog is on Piriformis syndrome


Piriformis syndrome is a condition which is believed to be a result form nerve compression, specifically at the sciatic nerve by the Piriformis muscle. Specifically it's a part of Deep Gluteal Syndrome. The nerve most affected by the syndrome is the Sciatic nerve, which is the largest and bulkiest nerve in the human body starting at 2cm wide and 0.5cm thick at its origin. The sciatic nerve is part of the roots of the lumbosacral plexus. Symptoms include pain and numbness in the butt and down the legs as such, sitting and running can cause symptoms to worsen.


Causes may include trauma to the gluteal muscle, spasms of the Piriformis muscle, variations in anatomy or an injury overuse. Few cases in athletics, have been described. Diagnosis of Piriformis Syndrome is difficult due to not having a definitive test, several physical exams, can be supportive, Medical imaging being one of them. Other conditions such as a herniated disk often present similarly.


Treatment often includes the avoiding of activities that involve movement, Physiotherapy, stretching and Medication such as NSAID's could be sued on those who don't improve, surgery is not a typical recommendation and the frequency of the condition being an unknown with various groups arguing about how common it is.


The history of Piriformis Syndrome is tied to the advancements in understanding the causes of Sciatica. In 1933, the proposal of disc herniation as a cause of sciatica shifted attention to the spine. Despite conceptualization as early as 1928 Piriformis Syndrome was incompletely understood and with a lack of clear diagnosic criteriam this syndrome was highly contorvercial. Advancements in medical technology including anesthesia, antibiotics and electrophysiology have led to revived interest in the syndrome. Advances in endoscopic surgery have led to discoveries which suggest a broader classification is necessary, this has led to the classification of Deep Gluteal Syndrome of which Piriformis Syndrome is just one cause.


Today Piniformis syndrome is in the same place a herniated disk once were. There's a link between Pathophysiology and the symptoms, which proves why the most studied treatments work, though Piniformis syndrome doesn't have a clear enough set of diagnostic criteria and a known prevalence for a set treatment. without a sufficient, high prevalence and a reliable diagnosis Controlled trials are unlikely to happen. The prevalence can't be determined without a reliable diagnosis method.
